  1.    What are your TOP two goals that you want to accomplish NOW?
  2.   What is your #1 goal that you want to succeed in 6 months from now?

  3.   Have you taken part in Pilates and/or Pilates Reformer training before?     

        If so, what is your background?

  4.      Any restrictions such as bad knees, slipped disc/nerve damage, surgeries,
